Attached is latest Membership Application with blocks for the 3 digit security code addressed below. This application will change 1 Jan 2011 when the E1-E4 membership dues rate changes to $25 for 24 months.
Please delete all previous version to include any applications you may have on your chapter website. With so many outdated forms floating around and being submitted to AFSA, they are having a difficult time processing them.
